OFFICIAL CONSUMER MAIL-IN REBATE FORM

To claim and track your mail-in rebate*, please follow these steps:

1. Purchase qualifying Hunter Douglas window fashions from your Hunter Douglas Dealer from
   October 5 – December 31, 2023.
2. Fill out this form legibly and completely. Only one coupon code, found in Special Email Rebate promotional emails, will be processed per household. If you make more than one qualified purchase, submit one rebate form with all dealer purchase receipts.
3. Mail a copy of your dated purchase receipt(s) along with your completed rebate form. The dealer should supply you with the dated purchase receipt when you agree to purchase in good faith, not at the installation or after. Dealers, designers, decorators, or those who purchase products for resale do not qualify for this rebate offer.
4. Mail the completed form and original, dated purchase receipt postmarked by January 31, 2024 to:

   Retain a copy of all materials for your records.
5. How to Check Status:
   Visit hunterdouglas.com and scroll to the bottom of the home page and click Rebate Status and follow the instructions provided on screen. If you do not receive your reward notification email within 6 weeks of claim approval, please check the status of your submission on hunterdouglas.com.
